Usage Guidelines

Purchasing a license provides users with:

  • A lifetime license (subject to the terms and conditions of the licensing agreement) to use the ORS and SRS in paper and pencil format
  • A file containing the official SRS and ORS forms that may be downloaded, distributed to and printed by users

NO ELECTRONIC OR DIGITAL USE OF THE SCALES IS PERMITTED.

Several authorized vendors provide systems for administering, scoring, and interpreting the scales. For more information on electronic versions, please visit the FIT Software Tools section.

While most agencies, organizations, and systems of care use one of the authorized electronic systems, licenses for paper and pencil use of the ORS and SRS are available to such groups for a fee. To determine the price, as well as register and pay, click on the Download Measures boutton above.  Use the drop-down box above to identify the correct number of providers within the organization. The fee and an option to pay will appear automatically.
